    WHAT IS THE PROCEDURE AND ELIGIBILITY FOR EMPLOYEES’ FAMILY PENSION CLAIMING WITHDRAWAL BENEFIT/ SCHEME CERTIFICATE : EPFO

    For Claiming Withdrawal Benefit/scheme Certificate, Form 10-C is required.

    Eligibility for Claiming Withdrawal Benefit/Scheme Certificate:


    1. Every member of the ended Family Pension Scheme 1971 and Employees Pension Scheme, 1995, who left their job before completion of ten years service and who has reached the age of Fifty eight years before achievement of ten years whether in job or left the job.

    2. Those member are also eligible who has complete 10 years job on the date of leaving the service and has not reached the age of 50 years on the date of filing this application or a member who has attained the age of Fifty years or more but less than 58 years and not willing for reduced pension.

    3. The family, nominee and legal heir of dead member who had died after 58 years age but had not completed their 10 years of eligible service is also eligible for Claiming Withdrawal Benefit.


    Kind of Benefits:Eligibility:


    <@>. A member who left their job before completion of 10 years service is eligible to apply to get the amount towards withdrawal benefit.

    <@>. A member who left their job before completion of 10 years service is not entitled to get Pension and he/she has to apply to get the amount towards withdrawal benefit.

    <@>. A member who has completed 10 years job on the date of leaving the service and has not reached the age of 50 years on the date of filing this application is eligible to get Scheme Certificates only.

    <@>. A member who has attained the age of Fifty years or more but less than 58 years and willing for reduced pension is eligible to get Scheme certificate by applying through Form 10D.

    <@>. In case of member, who died after attaining 58 years age but before completion of ten years of eligible service, withdrawal benefit to family or nominee or legal heir will be given.

    <@>. A member, who has left the service on account of total and permanent disablement, irrespective of his age and period of service, is eligible for monthly disablement pension and eligible to apply for pension through Form 10D.


    General Instructions for filling up the application form
    : (Sl. No. as given in the application form)

    {#@} Serial No. 1:
    Enter the name of member in Capital letters as mentioned in the service records of your establishment
    It can be furnished only when the application is preferred by a person other than the member himself (i.e. Nominee/ family member).

    The exact date of birth of the member should be mentioned

    {#@} Serial No. 3 to 7
    :
    You need to write clearly about the Particulars without any overwriting or cutting. If Correction made then be attested by the Gazetted officer.

    Member’s Account Number
    :
    Member’s Account number should have the a) Region Code in 2 alphabets b) Office Code in 3 alphabets, c) code number in maximum 7 digits), d) extension (sub code, if any, maximum 3 characters) and f) account number in maximum 7 digits.


    {#@} The Region codes has been changed after formation of the multiple regions in few states, they are:- Maharashtra, Tamil Nadu, Karnataka, West Bengal, Punjab, Gujarat, Andhra Pradesh, Uttar Pradesh, Haryana and Delhi.

    For getting the correct Region and Office Codes, you need to visit Establishment Search facility provided under link for Employees through the epfindia website ie. www.epfindia.gov.in.

    :
    This serial no. is made by the member only if he has not rendered ten years service and eligible for withdrawal benefit. In case of more than 10 years service member can only get the Scheme Certificate.

    {#@} Serial No. 9 and Serial No. 11: In case of option for Scheme Certificate

    Serial no. 9 must be filled
    Serial No. 11 not required to be filled

    {#@} Serial No. 9 and Serial No. 11: In case of Option for Withdrawal Benefit

    Sl No : 9: Is not required to be filled.
    Sl No11 : Should be filled in.

    The claimant should mention the Bank Account number on which the payment directly credited.. In case the bank account is in a computerized branch, payment may be made faster through electronic mode (NEFT etc). The claimant should attach a copy of blank/cancelled cheque ensuring that the IFS Code and Account numbers are clearly visible.

    Payment can be made through Money Order only if the total amount is less than 2000/-. The claimant should mention his/her POSTAL ADDRESS correctly with PIN Code so as to receive the money and any communication related to the claim.

    {#@} Serial No. 9, 10, 11 & 12: In Case Of Death Of The Member
    :
    Serial no. 9 : Details of the surviving family member/nominee/legal heir
    Serial no. 10 & 11 : Should be filled in by the family/nominee/legal heir.
    Serial no. 12: In case, the member is drawing Family Pension/Pension under the Employees’ Pension Scheme, 1995, the details should be furnished.

    Attestation Of Claim Form
    :

    #@]. This application form must be submitted through the employer under whom the member was last employed.

    #@]. In case the claim is through a form downloaded from the epfindia website, all pages should be signed by the claimant as well as the employer.

    #@]. In case of a closed organization whose Authorised Signatory or Employer is not available, then the applicant may forward the claims duly signed in the existence of any of the following authorized official and got attested with his official seal.
    · Head of Education Institution
    · Manager of the Bank where the member is holding an account
    · The Magistrate
    · A Gazetted Officer
    · Sub-Post Master
    · Member of Central Board of Trustees, Employees’ Provident Fund or Regional Committee.
    · Chairman / Secretary / Panchayat / Member of the Municipal / District / Local Board.
    · President of the village Union / Panchayat / Member of parliament / Member of Legislative Assembly.
    · Any other officer approved by the Regional P. F. Commissioner.

    Required Documents:

    Ø Succession Certificate in case of application by the legal heir after death of member

    Ø Must attach the Copy of blank/cancelled cheque

    Ø Death Certificate of member in case of death of member also needed to attach

    Ø Date of Birth Certificates of children of member, in case of applying for Scheme Certificate

    Ø Rupee one (1) stamp if choose for payment of Withdrawal Benefit through Bank.
